What would be in "placement" ??

I think the way that we specify the numbering could be improved (perhaps
a slider or something ? ). Not sure.

This doesn't sound like a suitable thing to have in this dialog IMHO. 

We'd probably be able to do the preview easily enough (dunno: does
xforms do rich text markup ?)

(Trivial: the dialog needs cleaning up in terms of layout)
