What may be happening is that LyX sees the error and stops without reporting the error or, more likely, LyX sees that the exit value of latex says somethings wrong but LyX doesn't know what and stays quiet. An extra latex run is what was required but as latex reported an error LyX never makes that extra call. So what we need is for lyx to report the error about multiply defined labels. Allan. (ARRae)
