Not me.  I have seen this also when importing 1.1.6 docs or it may
have been earlier 1.2.0cvs docs into 1.2.0cvs

Many of the ones I have seen really are empty.  No spaces just an
empty ERT inset.  At the time I figured it must have been something
dodgy about an earlier 1.2.0cvs I had used.
