A more leightweight approach to define the new types (using size_t &
ptrdiff_t from <cstddef> instead of size_type and difference_type from
<vector>).

The types are certainly large enough and there was a strong feeling that
the should be identical Standard-wise...

Would be nice if people with "unusual" compiler would check. It's certainly
ok on g++ 2.95
