I much prefer the second option because a framed outer box with an inner
makebox is senseless - LaTeX internally already adds the makebox in these
cases. Also for the same reason nobody would do this in ERT and I also have
never seen this in TeX files.
I'm not sure. Probably it would not harm to always offer it, and it
would make the code easier. But your points are valid. Either option
is fine with me.
I now implemented the second option.
