This can be done in several ways. If we exclude using Javascript etc,
here's one solution (there are more solutions):
If we add such subitems they should be clearly separated from the normal
items. I don't think it will improve usability though, because the menu
becomes more difficult to use.
Why is there a H1-header at the top of most pages?
That's not so good for different reason, better to keep with standard
practices here. If someone explains it's purpose, I can probably come up
with a more typical way of doing what's needed.
They can safely be removed.
