Attached is quite a large patch of small bits and pieces. A clean-up really.
The real change is that InsetIndex now behaves exactly as it used to.
Can now index-insert at the start of a paragraph and index-insert-last without
launching the pop-up.
Clean ups:
All insets derived from InsetCommand now have similar constructors based on
InsetCommandParams. All other c-tors have been removed.
small change to src/texrow.C (getIdFromRow) allows code to compile
src/frontends/xforms/FormXXX.[Ch]: implemented changes suggested by Allan
I have been testing this stuff quite hard. Don't think anyone will suffer!
