Our dialogs are not modal. They have vocation to stay open too :)

That's not the same. If you want a comparisaon, take the layout combo: you always want to know what is the current layout. Similarly, you always want to know where you are within your document. The fact that other dialogs are modal is just handy: you can have access to them easily and at your prefered position if need be. This is definitely not the same thing. Am I alone in thinking that way?

it to a toc/outline.

Map? IMHO, this term clearly indicate that it's a way to locate yourself in your document. Map == Carte (Geographique) but I guess you know that?

What is your definition of the Document menu?

Everything that touch the settings of a document. I even think that most of the "Document Settings" module should be available as stand-alone dialogs accessible through the Document Menu.
