time I succeeded to see the same that Neal reports. Create a math formula
\sqrt 2 and leave the inset. Come back to the math inset appending again
\sqrt 2 and leave the inset using the mouse. The previously entered
formula does not show there, come back and it shows again, and now for
good.
I know that it is strange, but that is what I see I have only drank

That sounds like the same thing I saw.
This time I openend a new, blank doc.
Type \c-m, \sigma^2 + x^2 + \sqrt 2. Click outside formula. This time,
\sqrt{2} stayed, but x^2 disappeared!
Still not reproducible here... (Windoze).
