Do I need a specific version of qt4 for this to work?
Debian currently ships qt4 v. 4.2.1

Yes. I can only get multiple lines at non-TOP dock with v 4.2.2. With
qt <= 4.2.1, all toolbars at non-TOP dock have to be put in one line.
