What I would like when opening a file is:
- if the file already exists in the current view, use the 'switch to
buffer or revert to disk version' dialog we have in 1.4.

The dialog is still there but there is some interaction problem right now:
- if the document is already opened and you choose "switch to buffer", the current view you are using will show you this buffer; and this independently from the fact that this Buffer was already opened or not in this view. This is because views do not own the stated Buffer, they are just a view of it. - if you choose "Revert to disk" then all the views of this Buffer will be reverted.

- Otherwise, just create a new view of the existing buffer.

Is it what we do now?

Otherwise we open the document in the window from which you asked for it (via the menu or the command buffer). Why would you want to open it in a new window?
