[PATCH] Custom Printing

I've attached my patch to fix the custom page size printing problems.
I've tested this myself and pretty confident it'll work properly.  It's
against the latest cvs but should apply okay to 1.0.4pre4.  So Garst can
you try this and see how it goes.

I've tested:
        view PS
        update PS
        export PS

with the following combinations of page settings:
        PageSize{A4}
        PageSize{Default}
        PageSize{Default} + geometry settings (margin etc)
        PageSize{Custom} + custom page width/height specified
        PageSize{A4} + custom page width/height specified.

and they all work okay.  At least according to how I've described my fix.
I won't commit this until Juergen (or anyone else who agrees with him) is
satisfied.
