Well, with the steps I outlined, we can write that support for 1.0 now.
Doing it for 1.1 doesn't make sense right now since a lot of things will
change.
However, thinking on 1.1, I'd like to hear more ideas on how to redesign
all the toc and reference stuff, in general. BTW the error report should
to be also redesigned.
