linuxdoc patch.

Hi,
        I have a patch related with the linuxdoc clean:

        * the latexdel inset is removed and replaced with the correponding insets
        * a new url inset is created, that deals with the url and htmlurl cases
        * all the insets have now a linuxdoc method, to write the linuxdoc output

        Is the aby one interested to aplly it to see if it works?
