Hi Usually, this issue comes from your version of MLT but there was a long time ago.
The solution is to update Mlt using this faq : https://answers.launchpad.net/openshot/+faq/1861 However, I'm not sure that will resolve your issue. I don't know why but I suspect an issue with your i386 version. Why you don't use the 64 bits version ? an Hardware problem ? If you can you should... Another track. Did you get this issue only with mp4 or with all types of files ? Did you use ffmpeg or his fork i.e libav or avconv ? .
