how to issue a FORMAT_UNIT command

Hi All,

Is there any utility to generate FORMAT_UNIT command ?
Also, on one of my scsi disk, the partition table is bad
and if I try to run fdisk on it to create new partitions,
the fdisk core dumps.

Any help is appreciated.
