Which kernel version do you use?
Kernel v4.7 with a few ib_srp and dm-mpath backports from kernel v4.8-rc1 and also a few SCSI patches that I'm still testing (see also https://github.com/bvanassche/linux/commits/srp-initiator-for-next).
