Did you make sure that asterisk answers your call on the FXO only after the
second ring?

Unlike in digital lines (PRI etc.), in analog ones, CallerID data is sent
between the first ring and the second ring, and not during the call setup
phase.

If you pick up the phone before the CallerID sending timeframe, you are not
going to  get it...

Also I know that there are incompatibilities with the different methods COs
send the CallerID over the line - every country has its own method of doing
that, and you should make sure you're using the correct one.

And one last thing - need to make sure you don't have noises on the line
that effects this. Some equipment is more sensitive than other. For example,
in the past, I've seen that CallerID gets scrambled if there's high activity
of DSL on the same line (I know it sounds weird, but it was reproduced every
time :))
