This lspci output doesn't help me since it doesn't really tells me what chip (and model) is inside your modem.
Looking at Intel web site, I see that they have Intel 536 and 537 models, and few others. If it's a PCI card, could you tell me whats the chip on the card? if it's PCMCIA, could you give the details whats printed on the card? if it's a USB, then what model/name does it have? if it's onboard, could you give the board name / laptop name+model?
