If you're actually using saa7134, and your question is related to the
insmod parameters:

.. that makes it 57 :-)
Check the video4linux/ folder in the Linux kernel sources
documentations, I guess that if you need any other information you'll
find it there.
2. How do I get MythTV to recognize it? I can't see any setup option
for tv capture cards.
I haven't work with MythTV, but I'm guessing that it works with devices,
check if you can just configure it to use "/dev/video0" or something
like that..
3. How do I get lirc to use the card as input and what remote do I
set it for?
LIRC configuration is based on text files. Any remote control has it's
own "configuration file"... i've found this, which has the information
and the files you need to configure your remote control using LIRC..
