Which ivritex exactly? Which version? Which tarball?

I assume that you used the -dist tarball of 1.2.1

I'm not sure exactly how MikTeX is set up, but this basically means that
it has failed to use Type1 fonts. Try debugging this first before
debugging the generation of .pk fonts.
