AFAIK ivritex doesn't include the david font by default.

I do use lyx, but it doesn't use david for documents under my setup.

Try at first to compile the attached very short latex sample to see if
ivritex installed correctly. That would help before debugging your lyx setup
(should be either "latex test_heb.tex" or "elatex test_heb.tex" should work
under windows from the command prompt if the path is set properly, chose
start->run and type cmd).
