Do you have a copy of tex (I guess it is tetex) on /usr/local?
If so - which of them is the one you use?
Where have you installed the hebrew latex files?
Have you tried running /usr/local/bin/elatex test-heb.tex ?
(Note that the outputs are created by default in the current directory, so
you are advised to run elatex from within ~/lyx)?
The hebrew latex files should be installed in the same copy of tetex that
you work with.
