It depends. In your main mailbox, for example, you can have lots
of unrelated messages with subject "Hi!", coming from different
people. Mutt can be configured without threading, with strict
threading (Reply-To: and References: headers only), or with
"loose" threading (like strict threading, but also using Subject:
header). For example, this thread looks like this in my mailbox:

The `*' characters mean that strict threading failed in this
case. As it doesn't know what message was replied to, it puts
the problematic messages on the top level.
So, the bottom line is that it depends on the situation what
threading scheme is most appropriate for this case. My main
(personal) mailbox is configured without threads at all.
