Windhover in Walpole is well known for its bird practice. I have often chickens there.
Dr.McMillan is well- established and respected through the vet community for this niche work.

I don't know of a poultry vet who does farm calls; but in case it helps.... I've found a couple of places with poultry knowledge:
Dr. Giles Siddons DVM
Lexington Vet Associates
511 Waltham Street
Lexington MA 02421
tel: 781-862-1127
email: i...@lexingtonveterinary.com
(did a necropsy on one of my hens)

Countryside Veterinary Hospital
N. Chelmsford
The following 2 places might provide useful advice:
MSPCA Angell
Jamaica Plain, MA
Their website doesn't list what species they cover but they seem to be able to consult with specialists for avian and exotic pets

UNH Durham  will do necropsies on chickens
603-862-2726
nhvdl.unh.edu
If you hear of someone else who handles chickens or farm calls, I'd love to hear about him/her.
