The command \markuplines has to be replaced by \markuplist. This change happened in 2.15.17. If you have a file with an older version and run convert-ly on it, then it will be corrected. If you claim this file to be 2.16.0, then convert-ly does not apply rules for 2.15.x or older versions.
Can you post the link where you found Booktest4.ly in the manual? If the 2.16 manual still has \markuplines in it, then the documentation is buggy.
