Difference between 2.12.3 and 2.14.2

The same .ly that gave me what I wanted at 2.12.3 is now 
giving me an extraneous clef and time-signature protruding 
from the top left corner of the first staff in 2.14.2.  Is 
this intended, and if so, how can I remove it?
