That's typically what I was asking for in my comment here: http://code.google.com/p/lilypond/issues/detail?id=684#c16 It seems the PATCH for #684 handles well placement of MetronomeMark when used simultaneously with RehearsalMark (AFAICS, based on Jan's example png). I hope this PATCH will be pushed soon, so we will be able to check this in next LilyPond version.
