I tried your idea, Mats, but for some reason, the notes above the plus sign
disappear when I add it (the plus sign).  Here's an example:

\markup { \column { \line \sans \override #'(line-width . 100) \override
#'(style . harmonic) { \hspace #5 LuftgerÀusch - \note #"2" #UP }
 \line \sans \override #'(line-width . 100) \override #'(style . harmonic) {
\hspace #5 Pizzicato - \note #"2" #UP }
\line \override #'(line-width . 100) { \hspace #17 + } }}
