Look at the options --png -dpreview -dno-print-pages -dno-include-book-title-preview in the page http://lilypond.org/doc/v2.13/Documentation/usage/Command_002dline-usage#Command-line-options-for-lilypond That can be used to print the first line in a PNG file that you can use for indexing. If you insist on the 20 notes, you could always force a carriage return in the right place when you're generating the incipits.
