That works perfectly for the clef change at the beginning of the piece.

Anyone have any ideas about the TrillSpan problem?

Have you tried \grace instead of \skip?
It adds the small complexity that you need to synchronize all Voice/
Staff contexts, but otherwise it has behaved perfectly for me...
