Looks like a nasty bug. I tried it with a recent 2.5.x version as well as with 2.4.2 and 2.4.6, and the problem doesn't appear there.
The next version 2.6 will soon be released, and the problem _might_ have been fixed there. If you upgrade to 2.6, please tell us again if anything similar ever occurs again.

(btw: the problem could be related to the ps->pdf generation process, I have seen displaced objects before. Could you check what the .ps or .dvi output look like? If they look good, a solution to your problem could be to download a newer or older version of ghostscript.)
