I assume you are using it wrong.

Always enter the sounding pitch in lilypond.  In the Notation Reference, they 
are showing you that if you don't change the pitch, an add an ottava, the note 
will displace in the opposite direction of the ottava bracket, because the 
pitch has not been changed.

Consider the following:

{
  c''4 
  \ottava 1
  c'' 4 c'''4
  \ottava 0
  c'''4
}
%%%%%%
I think this shows that \ottava works just as it should.
