The easiest way to obtain dynamics in parentheses is to use text markup. Something like c4_\markup{ ( \dynamic mp ) } or maybe you prefer _\markup{ \bold { ( \dynamic mp ) }
For note heads, take a look at the example called molecule-hacking.ly in the Regression Tests document included in the documentation for your version of LilyPond (please always tell what LilyPond version you use when you send a question, otherwise you might get an answer that's irrelevant to you, I have assumed version 2.4 above).
