Hi Jay, This is a known issue (that, unfortunately, has not been solved so far).
But there's a remedy: Acciaccaturas (and appoggiaturas and grace notes in general) at the beginning of a measure must be inserted in *all* the staves using "grace spacer rests" of appropriate duration. In your case, you can simply put a "\grace s8" in measure 3 of the upper stave: \grace s8 c8 \times 2/3 {d8 [c b]} g [d] <http://lilypond.1069038.n5.nabble.com/file/t3887/jay-acciaccatura.png> That way, timing will not get out of pace and the acciaccatura at the beginning of measure 3 in the lower stave will be positioned correctly.
