this part:
\override Accidental.stencil = #(lambda (grob)
(ly:stencil-translate-axis
(ly:stencil-scale -1 1) 0.3 X))
was actually missing the stencil.
In your snippet no Accidentals were called, so it remained undetected in 2.18.2.
I think in 2.19.x David implemented more decent error-checks/prints.
