The -fpdftex option to lilypond isn't very actively supported, so
Graham's suggestion is preferable. However it should say
ly2dvi -p filename.ly
otherwise you just get a PS file. ly2dvi produces the PDF file using
dvips followed by ps2pdf.

I don't know much (anything? :) about PDFTeX, but if you want a pdf, you can do
ly2dvi -P filename.ly
