I'm now guessing that there is a problem with 2.15.4. Can you use the
newly
compiled binary to compile key-initial.ly?
The newly compiled binary is the first I tried, see above: version
2.15.0
I have the translation branch in ~/lilypond-git
and the master branch in ~/lilypond-master
See your output above - it's 2.15.4. 2.15.0 is your installed version.
From your build directory run
/out/bin/lilypond lilypond-git/input/regression/midi/key-initial.ly
