Because mac users got used to manually installing python from macports or fink, manually editing the #!/usr/bin/python line, etc. In the case of lilypond-book, this isn't too surprising; only fairly technically-adept people want to use LaTeX in the first place. (and we're the only people using lilypond-book on texinfo, and AFAIK nobody *ever* runs it on html files.
