On Mac OS X 10.4.11 x86 double-clicking on the app does nothing.
Launching the binary from the command line directly gives:
~ $ ./Desktop/LilyPond.app/Contents/MacOS/LilyPond

The resulting .app is something you should be able to open with
open LilyPad.app
and should be similar to the current lilypond application.

I'm pretty sure Graham did this a while ago for me (like 2 years), but
I can't recover the results anymore.
