Found it. Section 8.1.3. I hadn't got that far :-) I think it should also contain a line along the lines of "for values of \musicglyph, see appendix C.4".
I don't know what you can do about it, but note also that C.4 looks dreadful in the pdf. Everything is shoved over to the right - so much so that the right-most column of glyphs is trying (and in some cases succeeding) to fall off the edge of the page.
