I'm sorry. The pasting XML I described earlier is wrong, and the real thing is:

I'm really sorry. I forgot I had QoS defined on the virtual machine.
And this problem doesn't happen every time. I encountered this problem when I
used the same XML to create about 100 virtual machines on a hypervisor.
