I found this new reproducer when I tried to work around the problem by creating
two separate documents. I thought that two smaller parts would be o.k. because
the problem first occurred when the document reached a certain size. So I
removed all the chapter with media, leaving only the comments and tables. Then
I noticed that the problem is still there. It is more obvious and easier to
reproduce (hopefully).
