Thanks Ben for all the information and updates.
Re CUPS printing dialog : unfortunately, LibreOffice uses this systematically,
and I suspect that therein lies the underlying problem. A conscious decision
was made to switch entirely to the CUPS subsystem for printing on Mac and Linux
rather than the lpd interface that used to exist. You might even be able to
find in earlier versions of LibreOffice a tool called "spadmin". This tool was
removed from the app bundles (I don't know why) for OSX to rely only on the
OSX-provided CUPS subsystem, albeit as a subset of the complete functionality
as far as I'm aware (in other words, a simplified UI dialog when displayed from
within LO). At some stage, further code changes caused the CUPS subsystem print
dialog to print Letter-sized pages with an offset on the Mac.
So, I'm pretty sure that this is a DUPLICATE of already reported behaviour, but
I will have to trawl through the bug database to try and find something. As I
know that this has been reported before, at least on the user mailing list, I'm
going to set this report to NEW for the time being.
