But it is not a bug. Each index must be updated individually. If you want to update all indexes simultaneously must use Tools> Update> All indexes and Tables (Herramientas> Actualizar> Todos los indices). In addition, in your sample document, index does not use the outline and it is normal that it does not refresh. I close the Issue. Feel free to reopen with more information if usefull.
