Yes, it's a common problem, here's why: Every call made through the phone company's analog lines is attenuated by a certain amount (I think it's 3db, but I don't recall completely). That amount helps normal calls to sound better (in theory). So when you make a conf call through the ksu, you have two calls through the phone company. You are person A, the first call is person B and the second call is person C. A to B is attenuated the normal amount A to C is attenuated the normal amount B to C is attenuate TWICE and they have a hard time hearing each other.
A chip was made for the TD1232 that boosted conference calls by 6db. I think the chip could only work on -4 and higher. I don't know if it is still made but there are probably people selling them on ebay or somewhere, even on this list. It wasn't perfect but it did help. Other solutions include getting 3-way calling from the phone company on certain lines; upgrading to the newer KXTDA system which has improved that feature and can support up to 5 or more on a conference; and lastly, what I often recommend, is learn to use one of the many conference services such as www.freeconferencecall.com They do a pretty good job if the conference members don't mind calling in to a third number.
