Maybe I was caffeine deficient at the instant that I read your message, but I had to look up the description of the modules:
KX-TD170 8 extension expansion KX-TD191 DISA KX-TD192 System Interconnect KX-TD290 PRI
From what I can tell from reading this list and your message, you are pushing the envelope for this list in at least two ways:
1. you have connected two KX-TD1232 cabinets
2. you are using T1 or ISDN-BRI or ISDN-PRI
and maybe:
3. you are using the DISA (many people just use the KX-TVS* integrated voice mail)
So, I suggest that you bug the local Panasonic service representative for the tech notes.
Here is a stupid thought -- Programming lets one assign any module to any slot. However, by default one places (and the default programming configures) the CO module in the lowest? slot, and the extension module in the other (upper?) slots. Maybe there is a bug, and the KX-TD290 will only inter-operate with the KX-TD191 when modules are installed in the default positions.
Since there is not very much usage of these features going on, all of the various combination might not have been tried (or even tested by the developers).

Anybody had any problems with the KX-TD290 (PRI module) coming on-line when having a TD191 (DISA) card in the same cabinet?
This is my current configuration: Master KX-TD1232-6: TD170, TD170, TD290, TD191, TD192 Slave KX-TD1232-6: TD170, TD170, nothing, TD192.
When I disconnected the TD191, the TD290 came on line.
