Ok, for the people who replied, thank you. It turns out that I had everything setup properly, but missed one thing. I had unplugged the old extension and had left the UCD Monitor running on it. I plugged the old extension back in, and turned it off, and it then worked on the new one.
Just to be sure we're talking about the same thing. the UCD monitor when enabled on a phone displays the number of calls waiting in a UCD queue. If I have the definition wrong, someone please correct me.
