Michael, 

We have our original 616 hooked up with SMDR and it works fine.  We have the identical 
cabling setup on a TD1232 so the hardware should be the same.

Have you checked to see if the 616 has the SMDR printing enabled  for all incoming and 
outgoing calls and set the page length, skip lines, etc ?  Somebody may have turned 
SMDR printing off on you. My 616 manual isn't handy, perhaps someone else here can 
give us the program number.

  I use an Okidata Microline 184 with the serial card option.  They still make

  them, and if you want to do call accounting, Panalog is a good choice.  I
  use 
  Tel Electronics Tel Sense.  It's a stand alone box which is extremely user 
  friendly, and hooks up to a Okidata or other serial printer.  It's a little 
  pricey, but beats buying a computer, and a monitor, and keyboard, and mouse.

  It's a little bigger than a stack of 3X5 cards.  Takes up very little space,

  and runs some very useful reports.  Check my website or Tel Electronics.
