Mike,
Is your leaf spring dressed to fit snugly in to the tail wheel assembly
"notch"? If not, there is a chance for side to side movement under load
and cause the bolt to loosen again. Just a thought.
My KR started life with a Matco assembly 15 years ago.

The assembly didn't look quite right and I finally determined the
pivot bolt hole was not drilled "straight" but was at a slight angle
side to side. I returned it for warranty and the new assembly I
received had a larger diameter pivot bolt. Not sure why the change. I
later converted to an Aviation Products unit to knock two pounds off the
tail.

When Matco came out with a 6" pneumatic wheel I had my tail wheel arm
lengthened about 1 1/4" to accept the larger wheel and that seems to be
an ideal setup, light weight assembly with 6" wheel. The only problem I
experienced with the pneumatic wheel was tire slipping on the rim and
causing the tube to go flat. Matco claimed I wasn't keeping enough air
pressure in the tire but I wasn't convinced. My solution was to coat
the tire bead and rim with glue from a tire patch kit and I've not had
another flat in two years, since Oshkosh 2017. Another thing, Matco
says if you have a flat you need to replace both tire and tube so they
won't sell the tube separately but again, I didn't find that necessary
as the tire never suffered any damage. I flew home from Oshkosh with a
flat tail wheel tire, two takeoffs and two landings with no tire or rim
damage. I found the same tube they supply to be a wheel chair tube and
purchased on line for $7.50 each.
That's my story and I'm sticking to it...........
